Camp Cultivate is a summer-long camp for rising 1st through 8th graders. Campers will participate in praise and worship, spiritual formation, academic enrichment, creative ministry, and recreation. The cost of Camp Cultivate is $50 per week. Family discounts and scholarships are available.
Registration packets are available at STREETS Ministries. Our staff will be on site to help you register on May 1, May 8, and May 15 from 5:00-6:30pm.

Ages
Camp Cultivate is for children who will be in grades 1 through 8 for the 2019-2020 school year. No exceptions will be made. Students will be grouped based on age for most camp activities.
Cost & Payments
All registration paperwork and fees for Camp Cultivate must be completed and submitted to the staff of STREETS Ministries (1304 N Graham St) or The Avenue Community Church. A non-refundable $25 deposit and this completed registration packet are required to reserve your child(ren)’s spot. The $25 deposit will be applied toward your total fee for the summer. Payments may be made by cash, check, or card. Make checks payable to The Avenue Community Church. Card payments may be made via phone (ask staff for more information if you’d like to make a card payment).

You may choose to pre-pay in full or submit payment each week. Your child will not be able to check-in to camp until full payment for the current week has been received. Payment for each week is due the Wednesday of the previous week. Full payment for the first week of camp (June 3-7,2019) is due on May 29, 2019.
Camp Schedule
Camp Cultivate will follow the general schedule shown below. There will be some exceptions for special activities and events.
Dates & Hours of Operation
Camp Cultivate begins on June 3, 2019 and runs through July 26, 2019. Camp Cultivate will be closed on July 4-5, 2019. The hours of Camp Cultivate are Monday-Friday from 8:00 am until 5:30 pm. Please do not drop off your child prior to the start time of camp.
Drop-Off and Pick-Up
Children should arrive between 8:00-8:30am each morning and must be picked between 5:00-5:30pm each evening. An approved parent or guardian must be present (with photo ID) for check-in and check-out each day. Children will not be released for check-out to anyone not listed on their registration form. Children will not be released to walk home without prior written approval from a parent or guardian.
Late Pick-Up Policy
A late fee of $1.00 per minute (per camper) past 5:30pm will be added to the fee for any camper who is picked up after 5:30pm (based on the time on the Camp Director or staff’s phone). Repeated late pick-ups may result in the removal of the camper(s) from the program, with no refunds for payments already made.
Field Trips
Camp Cultivate will facilitate multiple field trips throughout the summer. You will receive additional permission forms for each field trip. The waiver and informed consent form in this packet applies to these field trips.
Meals & Snacks
Children will be provided breakfast, lunch, and snack each day. If your child would like to bring their own meals, please be sure to pack all items in non-glass containers and label all items with your child’s name. Please do not pack any items that include peanuts. Due to grant funding for some camp-provided food, your child may be served a meal by camp staff, but your child may choose not to eat that meal.
Medical Needs/Allergies
Camp Cultivate staff are not permitted to administer medication to program participants. In the event of a medical emergency, Camp Cultivate staff will administer first aid in the best interest of the child. A parent, guardian, or emergency contact will be contacted if care is administered. Allergy medications may be administered if directed in writing by the child’s parent or guardian.
Special Circumstances
Parents and guardians are required to inform Camp Cultivate staff in writing of any special circumferences which may affect the child’s ability to participate fully and within the guidelines of acceptable behavior, including (but not limited to) any serious behavioral problems or special circumstances regarding psychological, medical, or physical conditions. Once the notice is submitted, a conference will be scheduled with the parent or guardian to discuss the special circumstances and whether our staff can accommodate those circumstances.
Dress Code
Children should dress appropriately for the activities scheduled. We recommend shorts, a light weight shirt or top, and athletic shoes or sneakers in order to participate in recreational activities. Campers may also want to bring a sweatshirt or jacket for indoor activities. Sandals, flip-flops, and other similar open-toed shoes are not recommended. Clothing that displays drugs, alcohol, tobacco, offensive language, excessive bagginess, or is excessively revealing will not be permitted. Your child must be dressed appropriately to check in to camp each day.
Personal Belongings
Please put the child’s name on all articles of clothing, snack bags, bags, etc. Children should not bring toys, mobile phones, electronic devices, jewelry, money, or any possession of value with them to any of the programs. Children will be responsible for their belongings.
Behavior/Discipline Policy
Camp Cultivate staff will create a fun and safe environment for participants in the program. Praise and positive reinforcement are used as effective methods of behavior management. In the event that children do not respond to these methods or are destructive to others or to property, camp staff will follow the procedures listed below. All incident reports will be discussed privately with parents/guardians and a copy of each report will be kept on file on camp premises.
In the event a child’s behavior is a repeated, disruptive behavior and cannot be corrected by the staff of Camp Cultivate with a verbal warning or other form of redirection, a first incident report will be written to document and correct behavior. A copy of the report will be given to the parent/guardian the same day as the incident.
A second incident report will be written if the behavior from the first incident report is repeated or if a new disruption occurs with the same child. This report will follow the same process as the first, but a one or two-day suspension could accompany this report, and no refund will be provided for suspended days. A copy of the report will be given to the parent/guardian the same day as the second incident.
A third incident report will be completed using the same process as the first two. The Camp Cultivate Director will write this report. The Director will provide a copy of this report to the parent/guardian. Dismissal from the camp program may occur at this time.
NOTE: Immediate dismissal from the program can occur at any time given severe circumstances. Refunds for missed days due to a discipline dismissal will not be granted.
